Patient Participation Group.

The next Patient Participation Group meeting is to be announced. These meetings are open to all registered patients so please come along if you are free. Your ideas and views are important to us and can help us shape services to our patient's needs.

 Appointments not attended

In the month of May a total of 17 patients did not attend their booked appointments (16 with the doctors and 1 with the practice nurse) nor were the appointments cancelled so they could not be offered to patients in need of them. If you no longer need or are unable to attend the appointment you have booked, please call the surgery on 020 7266 1449 to cancel it.

 

A Wellwatch clinic is held at the practice on alternate Wednesdays. The Health & Social Care Coordinator is available by appointment.

 

Wellwatch is a new service that aims to keep patients with long termhealth needs in Westminster area well for longer and help them avoid the need for hospital care. The patients are supported via face to face contact with one of our Health and Social Care Coordinators in the GP practices or over the phone.

Wellwatch supports patients who have one or more of the following long-term conditions:

  • Asthma
  • Chronic Heart Disease
  • COPD (Chronic bronchitis or Emphysema)
  • Depression
  • Diabetes
  • Heart Failure
  • Hypertension
  • Cancer (only via referral into the team and for those who are no longer undergoing treatments for cancer)

Exclusions: people under 18 years’ old, pregnant patients, those with active mental health psychosis or disease (so past or stable mental health problems are not excluded), patients undergoing treatment for cancer, those on the End of Life Care register.

The Wellwatch Team

The Wellwatch team, made up of GPs, a senior nurse, a health and social care coordinator and an administrator, will offer people healthcare assistance and direct them to community and voluntary care services that they might benefit from.
